LMSW / LCSW, managed care or Medicaid experience, bilingual (Spanish / Arabic / Chaldean), Motivational Interviewing.

Duration : 3-4 Months Contract
We're looking for a compassionate Field Care Manager to support members facing behavioral health challenges. This role combines clinical expertise with community-based care, helping individuals access the support, services, and stability they need. This is a remote position with 15–20% local travel within Wayne and Macomb Counties.
Key Responsibilities :
- Manage a behavioral health caseload and conduct in-home, community, or phone assessments.
- Develop individualized care plans addressing physical, social, and behavioral needs.
- Collaborate with interdisciplinary care teams, physicians, and caregivers.
- Help members overcome barriers such as housing, transportation, and benefits access.
- Serve as the primary point of contact and advocate for each member.
