What You’ll Do

• Participate as an active member of the interdisciplinary team, attending assessments and daily team meetings to share updates and advocate for participants.
• Develop personalized care plans after assessing participant needs for in-home care provided by Home Care Aides and / or Specialists.
• Deliver skilled nursing visits under physician orders.
• Respond to participant and family concerns with compassion and professionalism.
• Contribute to quality improvement initiatives and maintain accurate documentation.
