Valid driver's license, reliable transportation, auto insurance, and CPR certification. Preferred : 2+ years nursing experience, 1+ year case management, community health, and broad med-surg background. Competitive salary : $89,419-$106,620 annually. Mileage reimbursement or company car program. Generous PTO, holiday pay, 401(k) matching. Full medical, dental, vision, HSA with company contribution, scrubs allowance, and more. 100% clinical license reimbursement; up to $400 / year CEU stipend + 2 paid CEU days. Full-time, days (Monday-Friday) with holiday rotation (~2 / year) and possible weekend rotation based on staffing. Field-based role serving patients' homes in southeast Twin Cities metro; office in Roseville, MN.

RN Case Manager - Home Care in Minneosta
Join us and make a meaningful difference in patients' lives by delivering personalized, compassionate care right in their own homes. As an RN Case Manager , enjoy true autonomy and flexibility while leading interdisciplinary care for diverse patients in the southeast Twin Cities metro area.
Key Responsibilities
- Conduct comprehensive head-to-toe assessments, care coordination, disease / medication management, skilled wound care, catheter / ostomy care, infection prevention, and home safety evaluations.
- Visit patients in their homes to evaluate care quality, review clinical notes, and participate in ongoing interdisciplinary assessments.
- Implement physician-ordered services, educate patients / families on healthcare needs, and complete timely, accurate clinical documentation.
- Collaborate with the healthcare team to ensure seamless coordination, progress tracking, and discharge planning.
