PRN Home health Wound Care RN/LVN

This position provides in-home wound care and nursing services while ensuring the highest standards of clinical excellence and patient satisfaction. Responsibilities * Provide direct in-home wound care and clinical services in accordance with the individualized Plan of Care. * Assess, plan, and implement wound care treatments while ensuring continuity and quality of care. * Report changes in client condition promptly to the Director of Nursing and other appropriate personnel. * Educate and support clients and families regarding wound care, treatments, expectations, and precautions—maintaining dignity and confidentiality at all times. * Maintain accurate and timely clinical documentation in compliance with state, federal, and agency standards. * Participate in agency quality improvement initiatives and remain current with best practices in wound management and home health care.
