Palliative Care RN

This highly recognized health system is seeking a Palliative Care RN to deliver compassionate, person-centered care for patients and families navigating serious illness. The role is deal for a nurse who thrives on meaningful connection and interdisciplinary collaboration 💙.

The Palliative Care RN will be part of a hospital known for advanced heart, neuroscience, and oncology programs, with palliative services integrated across inpatient and ambulatory settings.

The position reports to the Palliative Care Nurse Manager and partners closely with the Medical Director of Palliative Care for clinical guidance. The Palliative Care RN collaborates daily with advanced practice providers, attending physicians, social workers, chaplains, case managers, and pharmacists to ensure whole-person care. Additionally, the position will support a consultative service that spans medical-surgical units, ICUs, and the emergency department, with occasional touch points to outpatient and home transitions.

The Palliative Care RN will conduct comprehensive assessments, manage complex symptoms, and guide goals-of-care conversations that honor patient values and cultural needs. The role will also will coordinate across disciplines to create timely care plans, ensure safe transitions, and maintain accurate documentation.
