Responsibilities:

• Conduct thorough assessments and provide quality healthcare services
• Collaborate with our interdisciplinary team for seamless patient care
• Ensure accurate documentation and timely follow-ups
• Facilitate smooth transitions for patients post-acute or skilled stays

Requirements:

• Current Nevada NP or APRN license
• Minimum two years of experience in primary care or geriatrics preferred
• Strong clinical skills and communication abilities
• Commitment to ongoing professional development and licensure requirements
• Positive attitude and dedication to patient-centered care
