Position Overview:
We are seeking a Board-Certified Primary Care Physician (Internal Medicine or Family Medicine) to join a collaborative clinical team in Oakland, CA. This full-time role focuses on delivering comprehensive, patient-centered primary care using evidence-based medicine and national practice guidelines.
The ideal candidate is passionate about preventative care, chronic disease management, and building long-term relationships with patients while promoting coordinated, cost-effective healthcare delivery. New graduates are encouraged to apply.
Key Responsibilities:
Provide comprehensive primary care services for patients with undiagnosed signs, symptoms, or health concerns
Focus on health promotion, disease prevention, and health maintenance
Conduct Annual Health Assessments (AHA)
Manage Chronic Care Management (CCM) programs
Provide counseling and patient education
Deliver direct and indirect patient care as part of individualized treatment plans
Identify actual or potential health risks based on screening data
Utilize Electronic Health Records (EHR) to manage workflows, documentation, and clinical compliance
Qualifications:
MD or DO degree
Board Certified in Internal Medicine or Family Medicine
Successfully completed residency in specialty
Active and unrestricted California Medical License
Active and valid DEA license
Active and valid NPI number
Minimum 1-2 years of clinical experience preferred
