About the Role:
Join Skilled Nursing Pharmacy as a PM Staff Pharmacist and play a crucial role in delivering exceptional pharmaceutical care to our patients. We are looking for a dedicated professional who thrives in a fast-paced environment and is passionate about improving patient outcomes.

Responsibilities:

• Review and verify medication orders for accuracy and appropriateness.
• Provide drug information and guidance to healthcare professionals and patients.
• Monitor patient medication therapies for efficacy and safety.
• Collaborate with the healthcare team to optimize patient care plans.
• Ensure compliance with regulatory standards and pharmacy policies.
• Participate in quality assurance and improvement initiatives.
• Assist in training and mentoring pharmacy staff and interns.
• Manage inventory and oversee medication dispensing processes.

Requirements:

• PharmD degree from an accredited pharmacy school.
• Active California pharmacist license.
• Minimum of 2 years of pharmacy experience, preferably in a long-term care setting.
• Strong knowledge of pharmacotherapy and medication management.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Detail-oriented with strong problem-solving abilities.
• Proficient in pharmacy software and technology.
