Schedule : 10-hour shifts, 4 days a week, including overnight, weekend, and holiday rotations.

Your Responsibilities :
Deliver direct patient care as per the State Nurse Practice ActImplement nursing practices based on comprehensive assessments and individualized care plansEnsure thorough and timely documentation in accordance with the care planProvide education and information to patients and families regarding diagnoses and care plansCoordinate hospice services by reporting patient needs to the interdisciplinary team, including aides, therapists, and supervisorsManage pain and symptoms effectively while evaluating patient responses to treatments and medicationsQualifications :
Graduate from an accredited practical nurse or vocational nursing programA minimum of three years of nursing experience; community health, hospice, or medical / surgical experience is preferredCurrent licensure as an LPN / LVNValid CPR certificationPossession of a valid driver’s license and access to a reliable, insured vehicleJoin us in making a difference in the lives of our patients and their families.
