Registered Nurse (RN) – Home Health | Scranton, PA | $38–$40 / hr

Schedule

Monday–Friday, 8 : 30 AM–5 : 00 PM

Occasional on-call, weekend, and holiday rotation

Position Summary

As a  Home Health RN , you will provide  skilled nursing care  to patients in their homes. You’ll perform assessments, develop care plans, educate patients and families, and help individuals safely manage their health at home.

Responsibilities

Provide one-on-one nursing care in personal settings

Complete initial and ongoing patient assessments

Develop and update individualized care plans

Communicate with physicians, caregivers, and families

Educate patients on health management

Accurately document all care according to home health standards

Requirements

Active RN license (Pennsylvania)

Minimum 1 year of Home Health or Hospice experience

CPR certification

Valid driver’s license and reliable transportation

Compassionate, dependable, and patient-focused
