North Carolina Licensed Case Manager

A company is looking for a Remote Case Manager.

Key Responsibilities :

Collaborate with a multidisciplinary healthcare team to monitor utilization patterns and facilitate appropriate care delivery

Conduct comprehensive assessments of members' health status, including clinical history and support needs

Coordinate with healthcare providers to develop and implement treatment and discharge plans

Qualifications :

Active RN license (North Carolina or compact state required)

3+ years of inpatient care experience, including critical care

Prior telephonic case management experience is a plus

Must reside in the Eastern Time Zone
