Care Navigator

A company is looking for a Care Navigator for a 100% remote, full-time position.

Key Responsibilities

Support current patients by answering questions and triaging them to appropriate resources

Document patient interactions and assist with administrative tasks

Conduct targeted patient outreach and provide feedback on processes

Required Qualifications

3+ years of experience in a healthcare setting, particularly in care navigation and patient support

Knowledge of healthcare terminology, especially in addiction medicine

Passion for serving individuals with addiction and mental health challenges

Experience with technology and common business tools

Ability to multitask and work independently in a fast-paced environment
