Job Summary :

To work within the Transition Support Services (TSS), which supports residents’ recovery from addiction and moderate to severe mental health conditions and prepares for reintegration to the community or to transition to their next level of care.

To be part of a Multidisciplinary Treatment Team that will be available to support the medical and pharmacological needs of the residents in the program and deliver services under the agency clinic license and in keeping with their supervisory requirements. The RN / LPN is one who actively promotes the agency’s philosophy and maintains a high level of commitment to the program and to the health and wellbeing of the residents.
