As a Home Health Registered Nurse, you will play a vital role in delivering personalized nursing care to patients in their homes, following physician directives and adhering to all relevant laws, regulations, and agency policies. Your responsibilities will include coordinating care with other health professionals to ensure a seamless continuum of care for your patients.

Key Responsibilities :

• Clinical Excellence :

Develop, implement, and update care plans in collaboration with physicians and healthcare teams.

Deliver high-quality clinical services, adhering to infection control and care plan directives.

Perform initial and comprehensive evaluations to confirm home health eligibility and establish care plans with physicians.

Work professionally under the direction of the Clinical and Executive Directors, completing tasks promptly, including necessary training.

Your compensation will reflect your experience, skills, and local labor market conditions, with a range of $28.27 to $50.48 per hour for full-time roles.
