Home Health Physical Therapist (PT) Opportunities in Pueblo, CO – Full-Time Available!

Compensation :

• $125,000 annual salary with $15,000 bonus

Bonus Structure :

Relocation Bonus : Up to $15,000 for candidates who reside outside of Pueblo, Colorado

Sign-On Bonus (Local Candidates) : $15,000 total , paid as follows :

$2,500 after 90 days of employment

$2,500 after 6 months

$10,000 after 12 months

Schedule :

Full-time

Monday through Friday

As a Home Health Physical Therapist, here’s a big-picture view of what you’ll do :

Provide in house physical therapy to patients who have lost mobility, strength and functionality due to an illness or injury

Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT, COTA, PTA and SLP, focused on the patient’s plan of care and goals

Observe movements, conduct assessments, document progress and modify care plan as needed

Assist patient with exercises and hands-on therapy to manage pain and increase mobility

Educate patients and family on their plan of care, therapy goals, home exercises, and self-care

A few must-haves for Home Health Physical Therapists :

Graduate of an accredited Physical Therapist Program and active PT license in Colorado

In the last five (5) years, two (2) years of experience as a physical therapist that is relevant to the patient population assigned preferred.

CPR Certification (demonstration course required)

Knowledge of state and federal home health regulations

Good clinical judgment, strong interpersonal skills, excellent time management and compassion
