POSITION SUMMARY

The Registered Nurse is responsible for assessing the health of individuals served, implementing and monitoring care plans, ensuring health maintenance and disease prevention, and providing case management as needed. This role requires strong clinical judgment, excellent communication, and the ability to coordinate care across multiple disciplines. This position covers

Mecklenburg and Union counties

and includes mileage reimbursement at

$0.41 per mile

KEY RESPONSIBILITIES

• Assess, observe, plan, implement, and evaluate nursing care for individuals in community or small facility settings.
• Work closely with interdisciplinary healthcare teams to plan and evaluate services based on patient needs.
• Ensure proper ordering, administration, documentation, storage, and disposal of all medications.
• Maintain accurate and timely nursing documentation, including daily, monthly, quarterly, and annual reports.
• Ensure physician progress reports are completed when patient conditions change.
• Transcribe physician orders and maintain comprehensive medical files.
• Maintain proper infection control standards for equipment, furnishings, and supplies.
• Schedule and follow through on all patient healthcare appointments.
• Coordinate communications between inpatient care teams and discharge planners.
• Supervise and provide direction to LPNs / LVNs as needed.
• Provide health and safety training for new employees and annual recertification.
• Train unlicensed staff on individual-specific health, safety, and disease-prevention procedures.
• Utilize preferred skills where applicable, including HCHB, wound care, IV / port management, wound vac care, and OASIS documentation.
