Job Summary and Responsibilities

As our Occupational Therapist, you will provide essential in-home occupational therapy services under physician's orders. Your role involves evaluating home safety, assessing functional needs, and implementing tailored treatment to promote independence.Every day you will evaluate home safety, assess functional needs of physically disabled patients, then develop, plan, and implement individualized occupational therapy treatment procedures.To be successful, you will demonstrate strong assessment, treatment planning, and implementation skills for in-home care. Excellent communication for patient education and a commitment to promoting independence and safety are essential.

• Appropriately assesses patients of varying diagnoses & disorders requiring Occupational Therapy according to established guidelines. Age range treated includes pediatric, adolescent, adult, & geriatric. Staff will be assigned only age ranges for which they meet education, experience & training.
• Patient's age 0-21 with CCS insurance will either be seen by CCS paneled therapists or have oversight by a CCS paneled therapist.
• Establishes functional goals in collaboration with the interdisciplinary team based on the patient's rehabilitation potential.
• Develops, implements & modifies treatment plan in collaboration with interdisciplinary team to achieve established goals. Utilizes evidence based practice as available. Incorporates patient / family / Care Partner needs, interests & goals in the development & execution on the treatment plan.
• Provides patient / family / Care Partner / caregiver education, initiating as soon as feasible after admission. Works to ensure transfer of knowledge using a variety of techniques (i.e. teach back, demonstration, video, written handouts).
• Establishes / follows patient precautions. Observes patient's response to treatment & reports unusual occurrences. Follows hand off guidelines.
