About the Position :

We are seeking a Family Nurse Practitioner (FNP) or Physician Assistant (PA) with pediatric experience to join our team in Hornell, NY .

Clinic Schedule :

  • Monday : 8 : 00 AM 5 : 30 PM
  • Tuesday Thursday : 8 : 00 AM 8 : 00 PM
  • Friday : 8 : 00 AM 5 : 00 PM
  • Work alongside two other APPs in a collaborative environment - 36 - 40 hour work week

Responsibilities :

  • Work closely with an MD / DO to lead the care team in medical decision-making
  • Conduct history & physical exams (HPI)
  • Diagnose conditions and develop personalized treatment plans
  • Prescribe medications following New York State guidelines
  • Perform minor surgical procedures
  • Provide patient education and referrals for specialized care as needed
