The Medical Needs Coordinator is to provide support for the Medical Needs Director and the Director of Residential Services in making sure that all residential staff comply with the rules and regulations of the Loving Hearts Training Center; in accordance with all the Person Centered Support Plans for the clients being served; Residential Staff Job Descriptions and all Federal and State regulations about the care of the disabled.
The Medical Needs Coordinator schedules all appointments with the clients' various doctors, takes clients to their appointments, assists in medical emergency appointments, and supports the Behavioral Management Review Process.
Key Responsibilities:
The Medical Needs Coordinator oversees and coordinates the individualized care and resources to the clients so that they may continue to live in their homes/apartments and be active out in the community. The care that is provided may include transportation to and from activities; to and from doctors' appointments; scheduling doctors’ appointments, assisting residential staff with house cleaning, personal hygiene; providing meals, and other health and wellness-related activities.
The Medical Needs Coordinator will ensure that all staff provide client support in a caring and respectful way by means of overseeing and performing Rights and Responsibilities training and Abuse and Neglect training. This training is done for all “new hires” and once a year thereafter for all associates.
This position also trains all residential staff in the understanding of a specific assigned client's PCSP (Person Centered Support Plan).
It is the responsibility of this position to communicate all medicine changes, additions, or deletions to the Medical Needs Director, Director of Residential Services, Site Manager, and all Residential and Day Staff to ensure that proper medications are being administered.
This position plays a major role in the development of the Behavior Management Review Meetings due to its expertise is the medications and medication changes for all the day and residential clients.
Failure to ensure that required services are performed fully may put the clients and others at risk.
This position rotates on-call duties once every 5 weeks to cover after-hours medical needs for clients
