The Associate Medical Coordinator plays a critical role in our clinical operations team by providing direct patient care and comprehensive assessments primarily in an ambulatory healthcare setting. Responsibilities encompass conducting geriatric assessments, reviewing medical histories, performing physical exams, diagnosing and treating conditions, developing care plans, referring to specialists as needed, and ensuring thorough documentation.

As an integral member of the healthcare team, you will demonstrate the ability to work independently while also collaborating effectively with various professionals. Your contributions will include participating in clinical rounds, engaging in continuous education, and adhering to our high standards for quality, service, and teamwork.

Key Responsibilities :

• Function as a primary care practitioner within a patient care team.
• Evaluate and address both acute and non-acute clinical issues.
• Conduct physical assessments, analyze patient data, and implement management plans.
• Plan patient care with consideration of individual needs, advocating for them at all times.
• Create and document admission, transfer, and discharge orders, along with lab analyses.
• Communicate effectively with team members regarding patient management strategies.
• Recognize emergencies and initiate life-saving procedures as needed.
• Foster collaborative relations with various healthcare professionals for optimal patient outcomes.
• Engage in professional growth activities outside of daily responsibilities.
• Ensure efficient resource utilization and strategic discharge planning for patients.
