Job Summary:

Astoria Healthcare Center is seeking a dedicated and compassionate Occupational Therapist (OT) to join our in-house therapy team! As a Full-Time OT, you will have the opportunity to work with a diverse range of patients in our Skilled Nursing Facility and Rehabilitation center. Our Rehab Department has an excellent reputation in the community for providing high-quality rehabilitation services, and we are committed to providing quality patient care with responsiveness, courtesy, and gentleness.

Responsibilities:

• Evaluate patients in a timely manner following physician referral

• Develop effective treatment plans and provide education for clients, family, and caregivers as a subject matter expert

• Communicate with supervisors and other healthcare team members regarding patient progress, problems, and plans

• Participate in in-service training programs for other staff in the facility

• Document all regulatory and clinically required information in the electronic documentation system and maintain patient records according to company policy

• Provide accurate and timely billing records and supporting clinical documentation

• Instruct patient's families or nursing staff in maintenance programs

• Coordinate necessary durable medical equipment and adaptive equipment for patients to facilitate safety and independence in ADL's

• Ensure resident abuse or neglect does not occur

• Observe confidentiality and safeguards for all patient information
