[job_card.job_description]Complete thorough medication reconciliation. Address open care gaps (fall risk, depression screening, cancer screenings, diabetic eye exams). Recheck elevated blood pressures and document preventative visit details in Epic. Assist with in-office testing (blood draws, mental status exams, EKGs, oxygen studies, point-of-care testing). Administer vaccines and injections within scope of practice. Schedule follow-up appointments per office policy. Collaborate with providers and clinical care team to monitor and coordinate care plans. Experience : 2 years’ hospital or clinical experience. Licensure : Graduate of an accredited practical nursing program. Consideration will be given for comparable education experience and successful licensure. Must obtain and maintain a current LPN license within the hiring state and meet all State Board of Nursing requirements. Education : High School Diploma or GED. Certification : Must obtain BLS certification within 90 days of hire. Preferred Experience : 6 months of clinical healthcare experience (physician’s office preferred). Education : High School Diploma or GED. Certification : Active MA certification / registration (RMA, CCMA, CMA) or EMT / Paramedic accepted. BLS certification within 90 days of hire.

Responsibilities include preparing patients for provider visits, performing clinical tasks, and supporting preventive care initiatives—all while delivering compassionate, high-quality service in alignment with Mercy’s mission and values.
