Clinical Generalist Pharmacist - Fort Wayne, IN

Location : Fort Wayne, Indiana

Job Type : Full-Time

Setting : Inpatient | Clinical + Distribution

Schedule : Rotational (Primarily Clinical : >

60% of the month)

Position Overview :

Seeking a dedicated and experienced Clinical Pharmacist to provide comprehensive pharmaceutical care in a collaborative inpatient setting. The role involves multidisciplinary rounding, medication therapy management, and direct support for healthcare providers. Opportunities available for mentorship and clinical teaching.

Key Responsibilities :

  • Participate in multidisciplinary rounds 7 days / week
  • Monitor, manage, and document patient-specific drug therapy
  • Advise physicians on drug usage, interactions, and appropriate therapy
  • Provide drug and patient information to support safe, effective medication use
  • Supervise and participate in preparation and dispensing of medications
  • Mentor pharmacy students and residents across various learning levels
  • Rotate through clinical and distribution responsibilities

Clinical Focus Areas :

  • Anticoagulation (e.g., warfarin, DOACs)
  • Antimicrobial stewardship (e.g., vancomycin)
  • Parenteral nutrition (TPN)
  • Pain management (e.g., PCA)
  • Other general and specialty inpatient areas

    • Technology Utilized :

  • Advanced EMR systems
  • Automated dispensing and inventory tools
  • Smart infusion systems and TPN compounders

    • Qualifications :

  • PharmD from an ACPE-accredited program
  • Indiana pharmacist license or eligibility
  • PGY1 residency completed or equivalent hospital experience
  • Strong clinical knowledge and decision-making skills
  • Excellent communication and teaching ability
  • Commitment to teamwork and continuous learning
