Social Services Designee – Mason Health Care Center | Cincinnati, OH

Mason Health Care Center, a small, boutique-style skilled nursing facility, is seeking a dedicated Social Services Designee to support our residents and families. This is a great opportunity to make a meaningful impact in a close-knit environment where residents’ well-being truly comes first.

What You’ll Do :

Coordinate and assist with admission, care, and discharge planning for residents.

Provide emotional support, resources, and guidance to residents and families.

Maintain accurate documentation and ensure compliance with facility, state, and federal requirements.

Work closely with nursing, therapy, and interdisciplinary teams to ensure smooth transitions and quality care.

Foster a positive environment that promotes residents’ well-being, adjustment, and quality of life.

What We’re Looking For :

Experience in a skilled nursing or long-term care setting preferred.

Strong communication, organizational, and teamwork skills.

Compassionate, resident-focused, and detail-oriented individual.

LSW license not required — we will train the right candidate with a passion for working with seniors.
