Life on dialysis can be a difficult transition for many patients-and you'll be there to support and advocate for them. In this vital role, you will help patients understand their rights and responsibilities, and guide them in managing the physical, mental, emotional and financial demands of End Stage Renal Disease.

Requirements for the Position :

Must have minimum LSW.

Master's degree in Social Work (MSW) required with a specialization in clinical practice

Demonstrated knowledge of government and private insurance programs

Basic computer skills in MS Word, Excel, PowerPoint and Outlook as well as functional proficiency with DaVita specific applications within 60 days

Essential Duties and Responsibilities :

Conducts, reviews, and assesses quality of life survey results, develops patient goal setting, completes appropriate follow up to analyze patients' plan to improve scores and quality of life.

Communicates with interdisciplinary team regarding patient psychosocial issues affecting treatment and wellbeing.

Counsels new and current patients and families on treatment options and modalities.

Conceptualizes problems, interventions and the evaluation of interventions with respect to human behavior, personality development, and social environment.

May be required to cover more than one facility in the area.

• Licensed or registered in the state of practice as required by state regulations. ACSW / ASW licensure required in the State of California.
