Description : Position Summary
Report to the Sr. Director of Behavorial Health & Addiction. Assist in the development, direction, organization, and coordination of various programs within the scope of Social Work. Coordinate activities for the development of the department; participate in planning departmental goals and implementing policies and procedures; assist in the development of new programs; also provide direct patient care service, staff supervision, consultation and serve on multidisciplinary planning committees and engage in related duties as assigned.
Duties & Responsibilities
Develops, manages, and evaluates clinical and operational initiatives and processes to support safe, timely, efficient transitions of care, throughput, and improved patient outcomes across the health system.
Plan, select and devise clinical operations, work methods, procedures, work flow and standards for post-acute and ambulatory referrals to programs that support complex patients.
Establish, implement and evaluate processes and metrics to support quality of care and more expedient transitions of care for complex patients.
Integrate shared leadership and foster collaboration across multi-disciplinary disciplines to arrive at decisions affecting the Division.
Develop and deliver training to health system regarding projects, programs, and new initiatives designed to address social determinants of health and complex patient needs at implementation and ongoing as necessary.
Advise, direct and review the work of subordinates and other clinical / ancillary personnel.
Monitor budget for assigned area or program and is responsible for adherence to approved budget.
Monitor and evaluate programs for compliance with licensure, accreditation and regulatory standards.
Determine personnel, material, equipment, and facilities needed to accomplish program goals.
Plan, assign and review work of staff to ensure that group objectives are met.
Hire, train, develop and manage staff to ensure that a qualified staff exists to meet group objectives.
Manage identified complex patient needs in collaboration with physicians, social workers, nursing, and external entities.
Manage community relationships and partnerships as it pertains to the skilled nursing facility and medical respite placement of identified complex patients.
Perform other related duties and participate in special projects as assigned.
