[job_card.job_description]Part-time and PRN (as needed) shifts Weekday and weekend availability Flexible scheduling Provide companionship and, if authorized, transportation to clients according to the care plan Assist client with general Activities of Daily Living (ADLs) such as bathing, meals, and safe ambulation / transfers Report any changes in the clients condition to the supervisor as outlined in the care plan Document care provided and maintain accurate records for the Agency Work collaboratively with the clients care team and ensure effective communication Promote client safety by following infection control practices and reporting unsafe conditions Attend in-service programs to meet compliance and notify supervisors of any educational needs Successful completion of a formal Minnesota certification training program or an Agency-administered training program that meets the requirements of Medicare Condition of Participation 42 CFR 484.80 Must be at least eighteen (18) years of age Previous experience providing care to adults in a home or facility setting preferred Good verbal, written, and interpersonal skills Licensed driver with automobile insured in accordance with state and / or Agency requirements and in good repair Current CPR certification Standards of Care : Our policies and procedures will follow industry standards and evidence-based practice, ensuring the best possible outcomes for our clients Collaboration : We will work closely with family members, physicians, other healthcare providers, or anybody else in the client-defined circle of care Over-communication : We will do our best to ensure our clients have all the information they need when they need it People First : We will prioritize people over profits, ensuring that their well-being, growth, and satisfaction are at the heart of every decision we make Transparency : We strive to be open and honest in all interactions, especially in times when our service has fallen short

We are seeking compassionate and dedicated Home Health Aides to grow with us as we expand into the Minneapolis / St. Paul, MN market. The ideal candidate is compassionate, reliable, and committed to providing personalized care to clients with chronic conditions, disabilities, or memory care needs.
