About You :

  • Doctor of Dental Surgery (DDS) or Doctor of Medicine in Dentistry (DMD) degree from an accredited dental school, or you are currently attending one to obtain your degree.
  • Have a current, valid dental license to practice in the state, or the ability to obtain one.
  • Possess DEA certification.
  • Hold a CPR / BLS certification or be able to obtain it.
