The WelbeHealth PACE program is dedicated to helping seniors maintain their independence at home by providing comprehensive medical care and vital community services. As a Home Care Program Coordinator, you will play an essential role by conducting in-home care assessments, laying the groundwork for our home health team to support participants in thriving at home.
Reporting to the Home Care Manager, the Home Care Program Coordinator will focus on organizing, assessing, and managing personal care in participants' homes.
Key Responsibilities :
Manage and coordinate incoming communication from participants, physicians, and agencies regarding medical orders, participant inquiries, and referrals.
Communicate effectively with participants over the phone and ensure seamless communication with nursing staff, therapy, aides, social services, and physicians about any changes in schedules, test results, etc.
Collaborate with Home Care Services staff to track and monitor home care schedules and service hours.
Work alongside the Marketing Team to assist with enrolling prospective participants in the program.
Support staffing and scheduling operations while gathering input from managers.
Contribute to end-of-life care coordination and provide necessary support.
Qualifications :
Healthcare / Medical Licensure or equivalent, along with a minimum of three (3) years of professional experience.
Bachelor's Degree preferred.
At least three (3) years of case management or nursing experience in a clinical or home care setting, specifically with frail or elderly populations.
Preferred nursing knowledge and training to address the complex clinical needs of frail, elderly participants.

Salary range for this role is $68,640 - $89,535 per year plus bonus. WelbeHealth offers a competitive total rewards package, including 401k match, healthcare coverage, and a wide range of additional benefits. Actual pay will be determined based on experience and qualifications.
