Job Description :

Foundation Medical Partners is seeking a compassionate and dedicated Geriatrician to join our team and provide Full-Time comprehensive medical care to our residents in a vibrant and supportive nursing home environment. As a Geriatrician with us, you will play a vital role in ensuring the well-being and quality of life for our older adult population.

• Conduct comprehensive initial and ongoing evaluations of residents' medical and psychosocial needs.
• Diagnose and treat a wide range of acute and chronic medical conditions common in older adults.
• Develop and implement individualized care plans in collaboration with nursing staff, therapists, and other healthcare professionals.
• Manage residents' medications, ensuring safe and effective use.
• Provide family members and caregivers with education and support regarding their loved ones' care.
• Participate in interdisciplinary care conferences and contribute to quality improvement initiatives.
• Stay up to date on the latest advances in geriatric medicine and evidence-based practices.
• Document medical records accurately and efficiently.
