Community Paramedics are delivering the future of healthcare by providing the right care, at the right time, in the right place. Providing an option for on-demand, in-home physician supported care, our innovative SmartCare division is improving patient outcomes, increasing patient satisfaction, and reducing the number of hospital admissions and re-admissions through community paramedicine.

Community Paramedics provide the highest levels of clinic assessment and treatment with an expanded scope of care. Patients are treated directly in their homes to reduce the need for ambulance transportation and hospital stays. Due to the nature of care needed, SmartCare professionals work as an extension of the patient's medical team and may work closely with the same set of patients and their families. SmartCare Paramedic Responsibilities

• Deliver quality patient care in accordance with clinical policies, statewide protocol, and standards of care
• Attend to patients in a courteous and professional manner
• Monitor remote patient tracking and respond as appropriate
• Participate in discharge planning of SmartCare patients
• Complete initial in-home assessments and on-going home visits as appropriate for all patients enrolled in SmartCare program
• Communicate effectively with EMS agencies, physicians, and other healthcare providers
