Job DescriptionCompany Description
We are seeking a Social Worker to join our team! You will provide emotional and administrative support to clients in either individual or group settings.
Responsibilities:
- Assess nature of participant's situations by interviewing and reviewing personal history
- Develop and execute individual treatment plans
- Determine appropriate milestones to gauge client progress
- Communicate client updates to all relevant parties
- Facilitate referrals to other healthcare professionals and programs
- Maintain accurate client documentation
Qualifications:
- Masters Degree in Social Work
- Masters in Psychology, Gerontology and Counseling
- BS in Social Worker with two years of experience in a healthcare or social setting.
Kingdom Living ADHC is a Community Based Adult Day Service Center. Our goal is to prevent unnecessary institutionalization, ER visits and hospitalizations by providing a full skilled MDT. Flexible schedule.
