Social Worker- Home Hospice

Per Diem - as needed

Day Shift (8am - 5pm)

This role works in collaboration with the Interdisciplinary Team to provide social work interventions, education, counseling and support services to address the identified psychosocial needs of patients and caregivers. MSW required, LSW or LCSW must be obtained within two years from date of hire.

We offer a full range of benefits including :

• Must work a minimum of 64 hours per two-week pay period

^ Must work a minimum of 40 hours per two week pay period
