Full-Time Licensed Practical Nurse (LPN) - Brain Injury & Short-Term Rehab - Northern Illinois
$15,000 Sign-On Bonus | Up to $40 / hr (Based on Experience)
Connected Health Care is a leading healthcare recruitment and staffing firm partnering with a respected facility in Northern Illinois to hire a compassionate and skilled Full-Time LPN for their Brain Injury and Short-Term Rehab unit. This is a great opportunity for an LPN who is passionate about rehab, patient recovery, and long-term outcomes.
Position Overview
As a Full-Time LPN in this Brain Injury and Short-Term Rehab setting, you will play a key role in helping patients regain independence and quality of life. You'll collaborate closely with RNs, therapists, and providers to deliver patient-centered, evidence-based care in a supportive, team-focused environment.
Key Responsibilities
- Provide direct nursing care to patients with brain injuries and those in short-term rehabilitation
- Administer medications and treatments in accordance with facility policies and physician orders
- Monitor vital signs, changes in condition, and promptly report to the RN / physician as needed
- Assist with wound care, ADLs, and rehabilitation-focused nursing interventions
- Support patients with mobility, transfers, and safe ambulation as they progress through rehab
- Document assessments, interventions, and outcomes accurately and timely in the medical record
- Collaborate with an interdisciplinary team including PT / OT, SLP, social work, and case management
- Educate patients and families on care plans, recovery expectations, and discharge planning
- Maintain a safe, clean, and compliant environment following all infection control standards
- Promote a positive, patient-first culture and uphold high standards of professionalism
