In this vital role, you'll be a guiding force for families navigating complex systems-connecting them to the services and supports that improve health, stability, and overall well-being. You'll build trusted relationships, coordinate care, and ensure that every intervention is both effective and compassionate.

Responsibilities

• Coordinate and oversee services for your assigned caseload
• Build strong, supportive relationships with families and service providers
• Connect individuals to essential resources such as housing, financial assistance, and substance use services
• Track outcomes and ensure interventions are impactful and client-centered
• Advocate for families and help them access the tools they need to succeed

Education and Experience:

• Bachelor Degree in Human Service or related field with two years of experience or
• Registered Nurse or Licensed Practical Nurse with one year of experience.

Additional Requirements:

• Relevant expertise and experience in serving children and families in child welfare, developmental disabilities, mental health, healthcare and/or other systems as well as those receiving preventive services.
• Experience providing service coordination and information, linkages, and referrals for community-based services.
