Description
SUMMARY :
The Care Coordinator, under the supervision of a Program Director, Assistant Program Director, or Lead Therapist, provides Camelot-approved therapeutic interventions, case coordination and / or resource linkage to clients and other involved parties (i.e. family, DCS, Court personnel, non-relative supports, etc.) to effect identified needed changes within the individual or family.

Therapeutic Foster Care
In addition to the general responsibilities listed above, the Therapeutic Foster Care Care Coordinator is also responsible for the following services : providing supervised visits and client-centered and strength-based individual, group, and family therapeutic interventions as outlined in the Treatment Plan; providing ongoing support and assistance to foster families; fostering sustainability by implementing discharge planning within the established time frames; participating in all necessary appointments for clients and families (school, DCS, court, medical, psychiatric, etc.); involving families in the treatment process to encourage reunification when possible; linking the client and family with specific services and resources necessary for success after discharge; monitoring medication management no less than bi-monthly while the child is in the foster home; and other duties as assigned, including any appropriate foster parent recruitment activities.
Prevention
In addition to the general responsibilities listed above, the Prevention Care Coordinator is also responsible for the following services : providing client-centered and strength-based preventative treatment and interventions to help clients avoid a higher level of care including state custody, juvenile detention, Care Coordinator I and / or hospitalization; establishing rapport with family members to effect identified changes necessary for sustained success; developing relationships within the community for both client and family resources, as well as referral resources; marketing to new referral sources and seeking out new opportunities for prevention services in the community; communicating with referral sources regarding client progress while maintaining confidentiality; being available to families in crisis; and other duties as assigned.
