Job Description

In this role, you will provide direct patient care, performing a variety of tasks that include medication administration, specimen collection, and clinical treatments. You will document patient care, monitor, and communicate patient conditions to the care team and families. Additionally, you will assist patients with daily living activities and collaborate across disciplines to coordinate care.

Responsibilities

• Perform plan of care interventions, including medication administration and clinical treatments.
• Document patient care provided and monitor patient conditions.
• Assist patients with daily activities such as hygiene, nutrition, and ambulation.
• Coordinate patient care across disciplines, including transfers and referrals.
• Evaluate patient and family learning needs, providing appropriate education.
• Advocate for the physical and emotional well-being of patients.
