MUST have ALL the documents listed below to be hired:

1. HHA and CNA License
2. Current CPR Card
3. Current Physical and proof of TB Testing/Chest X-ray (within the last 6 months)
4. Valid FL Driver’s License
5. Current Vehicle Insurance Card
6. Proof of Immigration status (Passport/birth certificate/green card, work permit, etc)
7. Social Security Card
8. Level 2 Background check done within the last 5 years.
9. CEUs
