What You Will Do to Change Lives

Under direct supervision, the Case Manager I and Case Manager II serve as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community.

Shifts Available :

Full Time | Tuesday - Saturday | NOC Shift : 11 : 00 PM - 7 : 30 AM

What You Bring to the Table (Must Have)

High School Graduation or G.E.D. equivalent; OR High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services; OR Associate's degree and one (1) year of experience; OR Bachelor's degree in Social Services

Valid and current driver's license, and personal vehicle insurance with your name listed as a driver

Willingness to use the company vehicle to drive clients to appointments and groups, etc.
