How youll contribute

A Mental Health Counselor who excels in this role :

• Essential Functions Effectively utilizes strong clinical and assessment skills cultural competence and basic casework skills (referrals advocacy).
• Provides therapy to assigned adult / geriatric patients per Behavioral Unit (BHU) policies developing and revising the treatment plan utilizing an interdisciplinary approach establishing specific short- and long-term goals. Identifies and addresses psychosocial cultural ethnic and religious / spiritual needs of the patient and patient representative.
• Documents the psychosocial evaluation all one to one and group activities therapies and case management activities in the patient(s) chart.
• Documentation is accurate and meets all hospital state and TJC requirements. Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical / health errors. Knows responsibility during a Code Gray and provides crisis intervention as necessary.
• Serves as a member of the interdisciplinary team participating in treatment team meetings evaluating and updating treatment goals and interventions.
• Provides / conducts group education and group programs per established schedule with emphasis on providing suicide prevention education to patients and / or patient representative
