[job_card.job_description]Spend quality one on one time with patients and their families to support everyday needs Care for hospice patients by providing paths, back rubs, oral hygiene, shampoos, and changing bed linen as often as needed Provide bedtime care including personal hygiene regimens as required Provide aroma therapy and gentle touch Provide lotion rubs to hands and feet Perform personal care needs after given a patient's assignment and attending to his / her requests promptly. If unable to perform a certain task, report to the Case Manager immediately. Provide patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality. Help patients dress and undress Record important health information including temperature, pulse, respiration, and blood pressure Assess and report patient condition and any significant changes to the Case Manager Assist patients with ambulation and exercise Respect patient and family / caregiver environment and patient's personal needs And additional duties as assigned Provide respite for patient's family / caregiver when on-site, as appropriate Maintain patient's living area is clean and orderly, as assigned Adhere to the organization's documentation and care procedures and standards of personal and professional conduct Participate in quality assessment performance improvement teams and activities Respect patient and family / caregiver environment and patient's personal needs All other duties as assigned Certification of Nursing Assistant or Home Health Aide required per state regulations Preferred to have one (1) year experience as Aide in community health / hospice or medical / surgical experience Must have completed a minimum of 75 hours of classroom and supervised practical training Licensed driver with automobile that is insured in accordance with organization / state requirements and is in good working order Willingness to submit to a criminal background check prior to hire and annually thereafter Current CPR certification Ability to read and follow written instructions An Understanding of hospice philosophy and comfortable providing specialized care to the terminally ill Ability to work autonomously with little direct supervision Successfully demonstrate tact, patience and good personal hygiene Ability to communicate effectively Strong organizational skills Understanding of plan of care documentation Flexible in work hours to ensure the patients receive care as provided in the plan of care Ability to empathize with the needs of the ill, injured, frail and impaired Comfortable addressing issues of death / dying Calm demeanor when entering the patients home Tuition Reimbursement PTO and Paid Holidays Medical, Dental, Vision, Life Insurance, and more HSA & 401(k) available Mileage Reimbursement for applicable positions Advanced training programs Passionate company culture committed to the highest standard of care in the hospice industry
