A company is looking for a Lifestyle Coach to support individuals managing Type 2 Diabetes through personalized coaching and digital tools.
Key Responsibilities
Deliver structured coaching sessions via phone, chat, and group formats
Guide members in identifying strengths, resources, and barriers while setting realistic health goals
Coordinate care with Registered Dietitians, Nurses, and Providers as necessary
Required Qualifications
National Board-Certified Health and Wellness Coach (NBC-HWC) or in the process of obtaining certification
1+ years of coaching / behavior change experience with proven results
Experience working with groups and individuals virtually
Experience in a healthcare environment regulated by HIPAA
Proficient in Microsoft Word, Excel, and Outlook
