South Mountain Healthcare in Vauxhall, NJ is seeking a full time Registered Dietitian to join our clinical team.
Looking for a Energetic, Dynamic and Experienced Registered Dietitian to work in LTC and Rehab environment.
Job Requirements
· The individual must be a Registered Dietitian with a minimum of 2 years of experience.
· Experience in long-term care preferred.
Job duties include but are not limited to:
· Must know Point Click Care (PCC) and Food Service software Meal Tracker.
· Completing timely nutrition assessments, developing and implementing nutritional care plans, documenting dietary information on residents, and well versed in doing MDS.
· Be proficient in Nutrition Care Process and electronic charting.
· Coordinates resident care with the Interdisciplinary team.
· Conducts meal rounds and interviews staff and residents to ensure residents are receiving foods in the amount, type, consistency, and frequency required to maintain or improve nutritional status.
· Inspects the food service area(s) with Food Service Director and practices for compliance with company policies, procedures, standards, and applicable federal, state, and local regulations.
· Participates in the long-term care survey process.
· Participates in menu planning and conducting audits.
· Provides in-service training to Nursing Center staff on topics related to Nutrition and Foodservice.
· Maintains all records and documentation according to Federal, State and Company requirements.
· Ensures exchange of information necessary with all departments as necessary for quality resident care..
· Ensures that all resident rights are protected.
· Must possess strong communication skill, be proficient in computer skills, possess strong customer service skills as well as have the ability to work with minimal supervision in a fast-paced environment.
