Bilingual RN Case Manager

A company is looking for a Case Manager RN - Level I - Bilingual.

Key Responsibilities

Manage an individual caseload using the Case Management process

Coordinate care transitions and collaborate with healthcare providers and teams

Participate in quality improvement initiatives and maintain documentation compliance

Required Qualifications

Current active and unrestricted Registered Nurse licensure in the United States

Bachelor's degree in a health-related field preferred

7 years of varied clinical experience preferred

Must be fluent in English and Spanish

URAC-recognized certification in Case Management within 3 years of hire
