Occupational Therapist, Hand Experience : 1 year Licenses : OT-CA(Copy needed) Certifications : OTR-National Board for Certification in Occupational Therapy(Copy needed)

BLS-AHA(Copy needed) SSN Required DOB Required Work History : Should not be more than 90 days of gap in last 1 year
