Social Services Director – Kane County, IL

A skilled nursing facility in Kane County is seeking an experienced Social Services Director to lead and oversee resident support services. This role is responsible for care planning, discharge coordination, psychosocial support, and compliance with state and federal regulations.

Key Responsibilities :

Develop and implement care plans in collaboration with interdisciplinary teams

Provide counseling and support to residents and families

Coordinate discharges and community resource referrals

Maintain compliance with IDPH and federal guidelines

Lead and mentor the social services team

Qualifications :

Bachelor’s degree in Social Work or related field (Master’s preferred)

Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) in Illinois preferred

Experience in long-term care or healthcare setting

Strong knowledge of IDPH regulations and care planning

Leadership and team management skills
