Location : Remote (Must reside in San Antonio, McAllen, or Dallas, TX)

Pay Rate : $18 / hour

Schedule : Monday-Friday, 8 : 00 AM-5 : 00 PM

Dean's Professional Services is actively seeking a reliable and detail-oriented Care Management Support Coordinator II to support administrative care management activities. This role includes outbound member outreach, inbound call support, scheduling services, and serving as a key point of contact for members, providers, and internal staff while ensuring accurate documentation and regulatory compliance.

Key Responsibilities

Provide outbound outreach to members via phone regarding care plan next steps, scheduling, available resources, and ongoing education

Support members in connecting with health plan and community resources to ensure high-quality customer care

Apply working knowledge of assigned health plan activities and available resources

Serve as front-line support for member and provider inquiries, including explaining care plan procedures and protocols

Support member onboarding and daily administrative tasks, including sending welcome letters, correspondence, and educational materials

Document and maintain non-clinical member records in compliance with state and regulatory requirements

Maintain knowledge of local benefits, community resources, and Social Determinants of Health (SDOH) to make appropriate referrals

Perform additional duties as assigned

Comply with all company policies, standards, and regulatory requirements

Qualifications

Education : High School Diploma or GED (Required)

Experience : 1-2 years of related medical or healthcare administrative experience (Required)
