Compact Licensed Bilingual LPN

A company is looking for a Bilingual Care Navigator (Remote LPN).

Key Responsibilities :

Complete phone consultations with patients, providing support and education about chronic conditions

Educate patients on health conditions and empower them to manage their chronic illnesses

Maintain accurate patient records and respond to remotely transmitted patient data

Qualifications :

Current, valid, and in good standing Multistate / Compact Nursing License (LPN / LVN)

3+ years of experience in primary care, cardiology, internal medicine, home care, or chronic care management

Proficiency in using electronic health records (EHR) and care management software

Reliable internet connection and HIPAA-compliant work area

Ability to work independently and collaboratively in a team environment
