Continuum Medical Staffing is seeking a Nurse Practitioner Hospice for a local contract job in Fresno, California.

'..Hospice & Palliative NP needed for 1-2 days / week with Pediatric experience. The NP will assist the Medical Director with face-to-face evals who require recertification on Medicare. Will communicate with the CM when any patient requires additional follow up. May round in the inpatient unit. May provide home visits to SNF, care facilities or homes. May educate clinical team members regarding care as needed.'
