Responsibilities may include :

  • Address the mental behavioral and physical health needs of clients on a weekly / biweekly and / or monthly basis.
  • Complete in person nursing assessments as well as medication and chart review in long term care facilities (LTC).
  • Educate caregivers and clients on their prescribed psychiatric and physical health medications and monitor for possible side effects.
  • Care management duties including coordination of community services and correspondence with treatment team members including physicians pharmacies employment programs.
  • Assist clients in building coping skills to address mood stressors and improve communication and in creating annual goals as it relates to their psychiatric diagnosis.
  • Possibly assisting clinic and residential needs for assigned locations.
  • Help train and mentor peers.
  • May need to cover more than one facility depending on company needs

    • Requirements for this position include :

  • Graduate of an accredited nursing program.
  • RN license.
  • Experience in psychiatric mental and behavioral health services.
  • Willingness to travel throughout assigned service area - Travel is required in this role
  • Preferably has experience working in LTC facilities as well as Behavioral Health.
  • CPR or Course can be provided during training
  • Experience in educating
  • Experience in creating and writing curriculum
